Register with us as a New Patient

All newly registered patients will be asked to complete the questionnaire in full pay attention to the questions. We require your previous GP Practice details and previous home address as this information allows us to proceed with your registration. It also allows our clinicians to gather as much medical information as possible, including medical history and details of current medications, until the practice receives your medical records. Patients who are found to fall into a specific disease category will be added to our register and will be cared for appropriately. Medical care is provided from the date you register although it can take some time for the practice to receive your full historical notes from your previous practice.

Under the current contract for general practice, you will not be registered with a specific doctor. Instead, you will be registered with Tanyard Muirkirk Medical Group.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that it will take 5-7 working days from the date you submit or hand the forms into the practice before you will be fully registered with the practice.

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
